The  Polk County Health Department has been in existence since 1979.  The services offered are available to all residents of Polk County, regardless of income status.  Funding comes from a number of sources including: private contracts, insurance reimbursement, private payment based on a fee for services, donations, grants and tax support from the Polk County Government.

 • Immunization Clinics - Adult and Childhood  

 • Drug/Alcohol Testing

 • Care Management

 • Lifeline

 

 

 

Homemaker/Chore Services

Services available include bath assistance, light house-keeping, laundry, meal preparation, prefill medication box, errands, travel escort, companion-ship, respite and others by agreement.  Fees for these services are covered by private pay, long term care insurance or medicaid waiver.

 

 

School Nursing

Nurses are available to schools on an interim basis for a contract fee. The contract fee includes the following services: classroom student CPR classes; student assessments; maturation classes; scoliosis screening; head lice checks; asthma protocol; bus driver drug & alcohol testing; immunization record checks; faculty/staff immunizations and health issues training. 

 

 

Health Screenings/Educational Programs

Some group insurance providers require a progressive health maintenance /prevention program.  Health Fairs conducted by PCHD at the job site can help a company meet their requirements. Public Health fairs, screenings, and health programs such as CPR/First Aid are available by request to any citizen or group in Polk County.  Scheduling for the above must be arranged in advance through the Health Department. The Educational Service Unit (ESU) contracts with PCHD to conduct annual school health screenings for all K-12 students in the school district.

 

 

Pedicure Clinics

Pedicure clinics are held at the Polk County Health Department in Osceola, the 1st and 3rd Wednesday  of every month. An appointment can be arranged by contacting the Health Department.  A fee is charged for this service.

 

 

Blood Pressure Clinics

Clinics to monitor blood pressure are held at the Polk County Health Dept in Osceola, the 1st and 3rdWednesday of every month.  No appointment is necessary. Walk-ins are welcome any time during office hours.  Please call first to be sure a nurse is available.

                                               

 

Influenza Immunization

Immunization against the influenza virus is recommended annually. Public clinics are held throughout the county during peak vaccination dates.

 

 

Adult Immunization Program

Tetanus and Hepatitis B immunizations are offered at the adult immunization clinic on the 3rd Monday of every month from 4:00pm to 6:00pm at the offices of the Polk County Health Department in Osceola.

 

 

Immunization Clinics

Polio, DTP, MMR, Hib, Hepatitis B, Tetanus, Prevnar, Meningitis and Chicken Pox immunizations are available to children age 0 thru 18 years. Monthly immunization clinics are at the following locations and dates:

  • Polk County (rotating sites in the four towns)

     - 2nd Wednesday of each month

  • Butler County, David City, NE - Wellness Center

      - 3rd Tuesday of each month

This program is partially funded through the Nebraska Department of Health.  There is no charge for the vaccine, however a minimal fee is requested to cover administrative costs.                                                        

 

Drug/Alcohol Testing

CDL holders need to be randomly tested for drug/alcohol use.  PCHD is a drug testing collection site for both non-DOT and DOT urine, and also has a certified saliva test technician to perform alcohol testing. Companies can contract with PCHD for these services.

 

Care Management

Care Management services are provided under the guidance of the Lincoln Information Service for the Elderly (LIFE).  The Care Manager’s main goal is to assist vulnerable elderly persons to remain in their homes and advocate for them to receive the services they need.

 

Lifeline

A personal response system that helps individuals to live independently in their home. If an individual has a sudden illness or injury, help will be on the way in a matter of minutes, even if they can’t move or speak.
