The Polk County Health Department is located on the west side of the Courthouse Square at 330 North State Street in Osceola.  The office hours are 8:00 a.m. – 4:30 p.m. Monday through Friday.

 

The Polk County Health Department has been in existence since 1979.  The services offered are available to all residents of Polk County, regardless of income status.  Funding comes from a number of sources including: private contracts, insurance reimbursement, private payment based on a fee for services, donations, grants and tax support from the Polk County Government.

 • Public Flu Vaccines

 • Immunization Clinics - Adult and Childhood
